In order to participate in athletics, all students must have a physical performed by a doctor. Physicals are good for one year from the date they are performed. Students must have two (2) forms completed: Preparticipitation Physical Evaluation and Medical History and the Athletic Acknowledgement of Forms completed and returned to the athletic trainer in order to participate.

The University Interscholastic League (UIL) and Troup ISD require that students/parents be give the following forms each year:

Parent/Guardian and student athlete sign the Athletic Acknowledgement of Forms CLICK HERE.
Step 3:
Complete the Preparticipation Physical Evaluation Medical History CLICK HERE. Both student athlete and parent/guardian to sign.
Step 4:
Bring the completed and signed Athletic Acknowledgement of Forms and the Preparticipation Physical Exam Form and Medical History with you on athletic physical exam day at the high school, or you may have physical performed by your own physician.
